Let me report back in for anyone else that may be looking in future...

Vintage speed replied. If you buy the mirror (part #W1103A) the mounting hardware comes with it (included). The mirror and rod cost $39.90 + $20 shipping domestic. ($59)

Beck appears to have a slightly nicer mirror with rod and all hardware for $69.90 +$10 shipping. ($79)
