Most cars I've seen do use the stock VW door pin and are accessed as David mentioned above.
On a Beck we drill an tap our door handles and use a long Alan set screw in place of the door pin. It holds much tighter and helps eliminate the slop in the inner door handles. On our early cars this was done underneath the escutcheon so you'd push the door panel back just like the pin style. Our newer cars we drill a 3/16" access hole on the bottom of the escutcheon so you can fish an Alan wrench in there without moving the door panel back.
